FAMILY MEDICINE & CLINICAL CARE CONTINUITY PACKAGE

The Clinical care continuity package includes:

1. annual check-up, one follow-up visit and one emergency visit,
2. review of your medical records and digitization of relevant information,
3. usually required laboratory tests for your gender and age group ,
4. priority appointment with the physician and/or counseling with the nurse,
5. access to telephone counseling with the physician (with charge),
6. renewal of prescriptions by telephone or fax,
7. filling out insurance forms ,
8. personalized assistance in making appointmentswith specialists or for follow-up &
9. annual influenza vaccination, if desired.

 CYTOLOGY SCREENING & SUPPORT

STD screening for men • $110

LABORATORY

Additional lab fees are charged according to the fee schedule of our partner DYNACARE GAMMA. Competitively priced they are eligible for reimbursement under most insurance plans.

We accept requests of laboratory test results from other doctors and clinics. In all cases, we will contact you to inform you of the nature of the results. In the event of a major abnormality, our nurse will make sure your doctor is advised and that referrals are promptly made as appropriate.

Note that Crea-MeD test results are obtained electronically by Gamma Dynacare and that they are automatically archived to patient records for better monitoring.

In the event you chose another lab, Crea-MeD will charge you to track, scan and manually archive your results to your electronic file. The cost of monitoring, archiving and digitization are $75 per set of laboratory tests performed on a given day.

DEPOSIT

A $75 deposit by credit card is required when booking an appointment. The deposit will be applied to services delivered during your appointment. In the event of cancellation, your deposit will be refunded to your credit card provided the cancellation is made at least one day prior to your scheduled appointment.
